There are 83 total collectibles in The Old Blood. 9 are the Nightmare Levels that let you play through Wolfenstein 3D levels, 64 are gold items (8 in each of the 8 chapters, with none in the prologue), and 10 are letters. Collectibles are saved as soon as you collect them, which means you don’t need to complete the level to keep the collectibles if you’re replaying it, and you can check your collectible progress from the chapter select screen.

In Rudi’s room, there is a skull you can interact with to have B.J. say “Alas, poor Yorick” in reference to Hamlet.
All nine chapters (counting the prologue) contain a room with a bed where you can rest to enter a “Nightmare” level that lets you play through the corresponding level from Wolfenstein 3D.
There is also a secret 10th Nightmare level. Near the end of the first Nightmare level near the elevator, you can find a secret room with a hidden elevator that leads to an additional secret level, the 10th level.
At the end of the prologue, there is a helmet sitting on the desk near the radio. It is the Dragonborn Helmet from Skyrim.
Vending machines found in the game include “Nuka Cola” from the Fallout series.
Wesler’s keychain, shown at the start of the prologue, features the Rocket Launcher from Quake 3.
In Chapter 2, you can overhear one Nazi criticizing another for not using proper grammar, a nod to the “Grammar Nazi” nickname for someone who is overly concerned with correcting grammar.
In Rudi’s room, there is a doll of a Cacodemon from Doom.
If you find all 64 gold items (collectibles), you’ll unlock the ability to wield golden pistols. Just open up the options menu and enable the “goldene pistole” option.
Keep your suppressed handgun ready at all times so you can dispatch enemies quietly when you need to.
When fighting regular enemies, try to get headshots. Efficient combat is key to success.
Perks are very helpful. The Eagle Eye perk is especially useful if you’re trying to find all of the collectibles, as it marks them on the map.
Break creates to get ammo, health, and more.
When you need to open the gate at the Docks, if you’re having trouble reaching the second lever, you need to swim to the other side to reach it.
