This is most likely a reference to the legend of King Arthur, who drew a sword from stone, or the story of the sword of St. Galgano, which can be found in Italy. While the King Arthur legend is better-known, the sword being located on top of a mountain makes it fit the story of St. Galgano better.

At the Obsidian Islands, if you climb to the highest mountain peak, you can find a skull and crossbones. Being a typical pirate symbol, this is most likely a reference to either Assassin’s Creed IV: Black Flag or Ubisoft’s upcoming pirate game Skull and Bones.
